The Requirement: Addenbrookes Hospital is part of Cambridge University NHS Foundation Trust. The Theatre department had been working with very old Anaesthetic trolleys that were no longer fit for purpose.Joseph Martinez, Clinical Leader, Theatres, explained, "The trolleys they replaced were more than 15 years old. They were here before I started at the Trust, so they were very outdated."The Trust wanted trolleys that could be used as Anaesthetic trolleys but also as airways trolleys and for both adult and children's applications. Using our LX Anaesthetic Trolleys as the base, they requested a custom drawer layout and additional security.https://vimeo.com/1032274620 Our Solution: One of the big issues with the old trolleys was that they weren’t tall enough to be used as a worksurface. Our trolleys solved this problem. Joseph commented, “The new trolleys were like a breath of fresh air.They were tall enough, so the Anaesthetists were happy to use them as worktops. They have so much more space, and we can store more equipment. So they are a massive improvement on our old ones."The moveable partitions allowed the staff to configure the drawers to suit their needs and the equipment they were storing.The previous trolleys did not have a locking feature, which was a priority for these new trolleys. Joseph explained, "The lockable drawer is a game-changer because we keep drugs in these trolleys. When the old trolleys were kept in the corridors, there was a risk that anyone could access them. Now they are accessible with a key, so we can safely leave drugs in the trolleys."We also customised the single-width trolleys with a tamper-proof drawer. If anyone accesses this drawer, the seal will be broken. This enabled the trolley to be used as an Airways cart.Our service doesn't stop once we have delivered the equipment. As part of our Listen | Design | Support approach, we encourage feedback from the people using the products and are able to respond quickly if any tweaks are needed. In the case of these anaesthetic trolleys, some were experiencing such high usage that they were causing static shocks. The Requirement Leicester Royal Infirmary and Glenfield Hospital are part of the University Hospitals of Leicester NHS Trust. The Hotel Services team had been operating with the existing clean linen carts for 15 years, so they were starting to show their age.This was causing some issues for the team, as Samuel White, Porting Manager explained, “The trolleys are quite old in terms of their styling. You have to physically pull the tow bars up and back down again. Some of the curtains are held on with string, so of course, you have to pull them across. However, they don't close properly, so in terms of infection prevention, they don't keep all the filling safe.”https://vimeo.com/1022001281 Our Solution The clean linen cart is the hospital's workhorse. It is a simple piece of equipment, but that doesn’t mean the design can’t be improved. We listened to the team's experiences using the carts and looked at how we could solve their issues.Issue #1 - The carts are heavyThe old carts were made from mild steel, which made them prone to rusting and very heavy to manoeuvre.We used aluminium instead, making the carts lighter and rust-proof. Samuel shared, “The night team have all said that they prefer the new trolleys because they are lighter to move and less heavy with the aluminium casing.”Issue #2 - Ineffective curtainsKeeping the cart contents clean and infection-free is essential, which is why the open sides of these trolleys have curtains. The old carts had curtains attached at the top, and many of the fixings had broken over time.As Samuel described, “Some of the curtains are held on with a bit of string; you have to pull them across. However, they don't close properly, so in terms of infection prevention, they don't keep all the filling safe.”The new trolleys have curtains that close on all sides, keeping the linen safe from dust and airborne germs.Issue #3 - The tow bar operation was awkwardThe old tow bar connecting the carts had to be raised and lowered by hand which required lots of bending up and down for the operators.Improving this was a vital part of the new design. As Samuel explained, “The hinge tow bar springs up, so you don't have to bend down as often to hook it up. You can do it with your foot, which is much safer and more practical from a manual handling perspective.”This project is an excellent example of Agile’s Listen, Design, Support approach.We listened to what was not working with the existing equipment and the client's specific requirements.We designed a solution that met their needs.We supported the client throughout the process with speedy and personal service. “Agile have a really fast response. The hospital was looking for 22 new carts to store ICU equipment, supplies and drugs. The Consultation ProcessIt was clear that an ‘off the shelf’ solution would not deliver everything the department needed. This is where Agile stepped in. As equipment manufacturers, we can customise our products and create something unique and highly specialised. Our Project Manager worked with the team at Croydon University Hospital to gather requirements and ideas.As Timothy Kuhn, Head of Nursing for Critical Care explained, “Designing the trolley has been a team effort. Now all airways equipment is in one drawer, which has made it so much easier to locate everything we need.All our hygiene items, such as linens and wipes, are in a closed drawer, which helps to avoid contamination. The lockable drawers ensure that it isn't easy for people to just dump items in the wrong drawer, which is really helping to keep them organised.”Easy communicationOne of the unusual features on the cart is the magnetic whiteboard. This acts as a two-way communication tool that allows staff to reference specific patient information quickly. It is also used to display information about the care team to the patient and even display family photos or items to help patients feel reassured and at ease.Additional securityOne of the issues with the previous cart was that it relied on users remembering to lock the drawers. And, in a high-pressure environment, sometimes this was missed, causing potential medication safety breaches. The locking feature on this trolley auto locks after 5 seconds, so it doesn't rely on people remembering, keeping drugs secure at all times.Improved efficiencyWith the trolleys in action, they are proving to be far more efficient and time-saving. Therefore, we increased the size of the top cavity for increased storage capabilities which housed individual compartments for organising the medication correctly. The medication compartment on every Exura cart is secured with a lock and key for security and safety reasons.Homerton University Hospital also realised the importance of having a service contract in place.
